Registration Form

To Join 3FC, you need to complete a Registration Form.

This form makes sure that

•     You have your parent's permission to spend time with us,
•     We know who to contact in the event of an emergency
•     We know about any special needs you might have, such as medical conditions
•     You agree to the rules of the club

Registration forms are available in the foyer of the Church, and a copy can also be obtained below, in PDF format, which you can print and complete.
